Mind Profile Psychology Clinic
As part of providing a service to you, Mind Profile consultants will need to collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the assessment and counselling / treatment / program that you will receive. These conditions of the services provided also apply to all consultants and staff involved in providing assessment, counselling or programs to you. We reserve the right to refuse or discontinue services. Prices are subject to change without notice.

Please note: We do not offer services under NDIS and are not able to assist with legal matters, workers compensation or insurance claims. Mind Profile does not offer crisis support. If you are experiencing an emergency, please call 000.

Privacy
Any personal information we get about you is strictly confidential. However, there can be exceptions, for instance when:

1. You or someone else may be at serious risk of harm
2. You have given permission to
   a) discuss your information with another person. eg. other professional, or
   b) provide a written report to another professional or agency
3. It is subpoenaed by court

If you have been referred by a GP please be advised we will need to correspond with your GP as required. Your signature below constitutes your permission for us to do this. By entering the Mind Profile Psychology Clinic, you consent and accept that the premise is under video/audio surveillance and that you may be recorded.

Access
As part of providing psychological services to you, we will need to collect and record your personal information. This information will be a necessary part of the psychological assessment and treatment. Your information may also be used for statistics or research to improve services.

Fees
The cost of a standard consultation (52 minutes) is $190 per session.
Medicare Rebate: $98.95 (Medicare conditions apply).
Private Health Fund Rebate: Please check with your fund if applicable.
Payment by cash, EFTPOS, Visa or MasterCard will be collected after your consultation.

Payment Terms
Payment for all sessions is due no later than the day of the scheduled session. Clients are required to finalise payment prior to or at the time of the appointment. Failure to make payment on the day may result in cancellation of future sessions or suspension of services until payment is received.

Medicare Mental Health Care Plan (MHCP) Responsibility
I understand that I am solely responsible for obtaining a valid and current Medicare Mental Health Care Plan (MHCP) from my GP prior to my session. I acknowledge that it is my responsibility to ensure my MHCP is in place and up to date if I wish to claim a Medicare rebate. I understand that Medicare claims will not be backdated under any circumstances, and any sessions attended without a valid MHCP in place will not be eligible for a rebate.

Cancelling appointments
If you need to cancel or reschedule your appointment, please provide at least 24 hours notice. Cancellations made with less notice may incur the full consultation fee, unless there are exceptional circumstances beyond your control.

Phone / Video / Remote Sessions
You must not record the session, copy it, distribute it or make any part of it available.
